If you're signing up for yourself, we'll be in touch with more info closer to the program date.
We are requiring the COVID-19 vaccine for all eligible groups. Please email/mail/drop off a copy of your child’s vaccine card. Please also provide a general vaccination list from your pediatrician’s office. This could be a printout from an online health record or an official list from your doctor.
Additionally, please download and complete the forms listed below. Completed forms can be emailed to us at tnlsteam@gmail.com or printed out and brought in person when you arrive at the NatureLab. Thanks!
MMA Form
If your child requires medication (for example, allergies requiring an epi-pen), please also prepare and submit this form.
Asthma Action Plan
If your child has need of an asthma action plan, please also prepare and submit this form.
